Healthy Heart CHF Program

 

The Healthy Heart Program at Assisteo provides comprehensive patient education, identification of lifestyle risk factors, and disease management to clients that suffer from congestive heart failure. Our goal is to increase patient compliance with dietary, medication, and lifestyle changes thereby reducing the hospital readmissions. Our highly skilled team of clinicians are dedicated to assisting patients living with heart failure to manage CHF successfully at home.
