Home Health Qualifications
For Medicare, Medicare Advantage Plans, and AHCCCS plans:

To qualify for home health services the patient must meet the following criteria:
1) Have a current following provider (can be a physician, nurse practitioner, or podiatrist) who agrees to sign the home health plan of care.
2) Must be homebound. This does not mean bedbound. It must be a taxing effort for the patient to leave the house and requires assistance. Driving is not permitted.
3) Must have a complete home health referral (including signed orders) sent to the agency.
4) Must have a skilled need on an intermittent basis. A skilled need is defined intermittent skilled nursing care (other than solely venipuncture for the purposes of obtaining a blood sample), Physical therapy, Speech-language pathology, Occupational therapy, and Medical social services.
