Assisteo Home Health

Home Health is a leading provider of Medicare Certified Home Health Services that enables adults to be cared for in their home environment that is warm and familiar. Our highly qualified team clinicians: Nurses, Physical Therapist, Occupational Therapist, Speech Therapist, Medical Social Workers, and Home Health Aides, provide the highest quality care with compassion, commitment, and integrity. Our locally based office works with Medicare, most major insurance plans, managed care organizations and AHCCCS plans.

Assisteo Hospice

At Assisteo, our mission is to provide compassionate, end-of-life care, addressing the physical, social, spiritual, and emotional needs of our patients and their loved ones. With an emphasis on symptom management, our hospice team members will develop a plan of care centered around you and your family’s goals.  As we join you in your journey, our hopes are to brighten each day, adding comfort and quality along the way.

Assisteo Palliative

The goal of Palliative care is to improve your quality of life while still receiving treatment or home health services. Our Palliative team will help manage your medications and care plans in conjunction with your current providers. Our team will make sure that all your doctors know and understand what you want regarding your treatment. This gives you more control over your care and will improve your quality of life.

Teri Ellerbe

Administrator

Teri joined Assisteo in 2012 and comes to us with an extensive healthcare background. Teri was Director of Medical Management at both Health Choice and Care 1st Arizona for a combination of 9 ½ years and has 10 years of home health experience with Visiting Nurses Association. She creates the clinical content and workflow applications to support the wellness, tele health and population management programs that the company manages. Teri has played a key role in the rapid growth and development that Assisteo has experienced in the last 2 years. Teri is married with 2 children and enjoys spending time outdoors hiking.

Cynthia Cloutier

Director of Nursing, MSN, FNP-C

Cynthia Cloutier is the Director of Nursing at Assisteo Home Health. Cynthia has many years in leadership, extensive knowledge in the home health industry, and has trained at The Joint Commission in Illinois. She brings with her vast knowledge with The Joint Commission, CMS, and state rules and regulations in home health. Cynthia has been the Director of Nursing in many successful Joint Commission surveys, ranging from one to five days, resulting in deficiency free results for organizations. Cynthia prides herself on communication which in turn significantly effects quality patient care and it is evident in her role here at Assisteo Home Health. Cynthia is a key part of Assisteo’s business development. She is involved in the creation and implementation of our specialty programs and presents these to potential referral sources.

Lisa Foster

PT, MPT COS-C

Lisa serves as the Director of Rehabilitation Services, managing a clinical team of over 35 physical therapists, occupational therapists, speech therapists and their corresponding assistants ensuring that they maintain the highest of industry standards. With over seventeen years of experience in physical therapy and over 11 years in therapy administration, Lisa has spent the majority of her time in the sub-acute and home health settings, working with diverse patient populations ranging from neurological, orthopedic, general debility, post-surgical, and wound patients.

Lisa received her Bachelors of Science in Business Administration from the University of Arizona. She then continued her education to receive her Masters in Physical Therapy from the Mayo School of Health Related Sciences at the Mayo Clinic in Rochester, MN.

Sue Hilles

Operations Manager/Human Resources

Sue Hilles brings over 15 years of experience and a diverse background in healthcare to Assisteo Home Health. As the Operations Manager, Sue is focused on managing the day to day operations within the home health agency. Before joining Assisteo, Sue has served as the Operations Manager in home health, managed care utilization and enteral feeding. Sue has experience in contracting, credentialing, recruiting, network development and management, Medicare/AHCCCS, and Joint Commission rules & regulations, and a wide knowledge of healthcare. Sue is known for charismatic motivation of the team, and is able to sustain solid relationships with business partners to improve the overall profitability of the business. Sue also holds certificates in Human Resources, State of Arizona notary, AZCHER and EMS Safety as an instructor for CPR/First aide.

Christy Ervin

Director of Business Development

Christy is a Director of Business Development with over 19 years of experience in health care. She worked as a discharge planner in the valley for over 5 years before transitioning to Home Health. Christy graduated from the University of Louisville with a degree in Psychology which has served as the foundation for her success with providing impeccable service to her patients, referring doctors, hospitals and rehab facilities. With her extensive experience and knowledge base of the home health industry, she is able to assist patients and their families with community resources, patient advocacy, and patient transitions to return home or a more suitable step down environment.

Linda Knapper

Community Liaison

Linda Knapper is a Liaison with Assisteo Home Health. Linda worked previously as an Occupational Therapist in home health for 15 years which also included her role of Rehab Coordinator. Linda then began a role as a liaison for home health, proving that her home health background was beneficial by relating to both patients and families. One of Linda’s best attributes is advocating in the needs of the patient and facilitates this, which creates a better way to serve case managers. Linda is married with three grown boys and enjoys taking classes at the local YMCA, hiking, and traveling.

David England

Community Liaison

David England is a Marketing Liaison Associate at Assisteo Home Health. He brings five years of medical background including home health, vascular and echo ultrasound, and fifteen years of sales experience. His priorities are patient care, communication, and customer service. David began his home health career in intake and staffing several years ago which provided the foundation needed for his skills in marketing. He also lead the team through state surveys as Interim Administrator for the agency. David spends his time off with his family and two dogs. He also enjoys working on classic cars and traveling.
