Diabetic Program

Assisteo Home Health is committed to providing patients with Type I and Type II Diabetes comprehensive in home diabetic care. Our proactive multi-discipline approach will focus on personalized care, education, prevention of comorbid conditions, and patient program participation. Our goal is for patients to gain an understanding of their disease process, obtain ongoing support from our Registered Nurses and Therapists, and to become independent in the self-management of their diabetes.

• Physician electronic signature and patient monitoring

• Specialized wound treatments based upon the type, location and size of the wound.

• Recommendations for support surfaces to promote wound healing and prevent  complications

• Advanced treatment options such as Wound Vacs

• Ostomy care with individualized ostomy care protocols

• Nutritional counseling if appropriate

• Coordination with the patient’s physician and wound clinics for treatment enhancement options
